DVA Gold Card Holder Consultation

National Disability Insurance Scheme (NDIS)
Fee’s incurred for services provided within the NDIS align with the National Disability Insurance Scheme (DNIS) Price Guide.
To access dietitian support under the NDIS, you will require funding in one of the following categories:
- Improved health & well-being
- Assistance with daily life
- Improved daily living skills

Online or Teleconferences
Online and tele consultations are also available for those who are unable to attend the clinic. Please note that rebates (Medicare and DVA) are not available for these consultations.
We accept cash, eftpos and credit cards. Payment is required on the day of consultation. Transaction fees may apply.
With some chronic conditions, your GP may choose to refer you to for treatment by other health care providers through a Chronic Disease Management Plan (or Enhanced Primary Care Plan). If this is the case, you are eligible for a rebate under the Medicare initiative. Currently, dietitian rebates are $53.90 a session and you will incur a gap fee. Discuss your eligibility for this referral with your doctor and bring a copy of your plan to the session
Private Health Funds
Benevolent Nutrition Services is registered with private health funds and you can claim rebate back after your appointment. We will provide you with a receipt to make claim. Check with your private health provider before visiting the dietitian to verify eligibility.
Benevolent Nutrition Services is a provider for the Department of Veteran Affairs (DVA). If you have a Gold Card, you are eligible for FREE dietitian visits (including home visits). If you have a White Card, you may be entitled for a partial or entire rebate for treatment of a specific condition. Please contact DVA to verify your entitlements. If you are DVA eligible, please see your doctor for a referral before visiting the dietitian.
Please provide 24 hours’ notice if you unable to make a scheduled appointment. If you failure to attend an appointment without 24 hour cancellation, you will be charged the full cost of the appointment. We do appreciate, however, that short notice is unavoidable in some circumstances. Discretion will be exercised in such cases.
*Rebates are available for eligible patients with an Enhanced Primary Care (EPC) plan (currently $52.95 rebate). Eligibility is determined with your GP. Under the EPC scheme you may be entitled to 5 bulk-billed or rebated sessions with your dietitian.
**All DVA gold card holders with a D904 referral will incur no charge for dietetic consultations. Home consultations are available for gold card holders experiencing difficulties attending appointments or with special needs.
***Private home consultations can be arranged for private consultations. This fee captures a 60km radius from 4564.
